Sometimes you may complete a note and later notice that it appears missing from the chart or shows up as unwritten. This usually means the note isn’t linked to its session. The good news: this is easy to fix as long as the note is unlocked.
This guide walks you through how to relink a note so it appears correctly in the client’s chart.
Before You Start
A note must be unlocked and unsigned to make any edits.
If the note is locked or signed, submit a request to Clinical Admin via the platform chat to have it reopened.
Once the note is unlocked, you can proceed with updating the session details.
Why Notes Go Missing
The platform links notes based on the session’s date and time.
If the note’s date/time does not match an existing session, or if the session was moved after the note was created, the note will appear as unwritten or may not show in the expected location.
Correcting the session date/time will resolve this.

How to Link (or Relink) Your Note
1. Open the note
Find the note that appears “not in chart” or is missing from the client’s session list.
2. Click “Edit” (top right)
This will allow you to change the session details. You will only be able to edit notes that are unlocked/unsigned. Please reach out to to Admin to unlock/unsign notes.
3. Update the session date and time
Change the session date and session time fields to match the actual date and time of the session that was scheduled.
Once you save the updated date and time, the system will automatically link the note to the correct session.
You do not have to manually attach the note.
4. Save your changes
The note should now appear under the correct session in the client’s chart. Sign and lock the note.
If You Still Don’t See the Note
If the note does not reappear where expected, or if you cannot access edit mode because the note is locked:
Contact Admin for support.
Include:
Client ID
Date of session
Date of written note in chart
