Risk Identification & Emergency Resources - Jan 1, 2026

Platform now automatically identifies clients who may be at higher risk based on their Therapy Intake Form responses and proactively shares emergency resources β€” ensuring timely intervention for both the clinical team and the client.

πŸ” How Risk Is Identified

Risk is evaluated when a Therapy Intake Form is submitted for the first time. The following criteria trigger a risk identification:

Intake Form Triggers

Case

Condition

Case 1

For the question "Have you ever been hospitalized for your mental or psychiatric care?", the client answered "Yes, in the last 3 years" AND for the suicidal ideation question, the client answered "I've had some thoughts of suicide"

Case 2

For the suicidal ideation question, the client answered: "I've had frequent thoughts of suicide", "I think about suicide on a daily basis", or "I think about suicide multiple times a day"

PHQ-9 Triggers

Case

Condition

Case 3

For the question, "Thoughts that you would be better off dead, or of hurting yourself in some way", the client answered: "2 - More than half the days" or "3 - Nearly every day"

πŸ’‘ How It Works

    • Therapist & Clinical Leadership receive an email notification

    • Client receives emergency resources (if not sent in the last 6 months)

    • PHQ-9 form displays emergency resources blurb (if Cases 3 or 4 apply)

  4. High-risk task force ensures the case is tended to promptly
