HLC Form
The following criteria must all be met prior to a client being informed of, or referred to, external (non-Ruh Care) higher level of care (HLC) services:

Criteria
The provider has clinically determined and documented that all Ruh Care Services have been exhausted and unsuccessful in meeting their client’s needs.
The provider has clinically determined and documented their client meets, at minimum, one of the following criteria:
Acute inpatient hospitalization is clinically required
A crisis stabilization unit is clinically required
Partial hospitalization service is clinically required
Intensive community-based service is required
An intensive outpatient program is clinically required
An intensive eating disorder program is required
An intensive addiction program is clinically required
The client is unable to access needed levels of care at Ruh Care
(Note: A client’s diagnosis alone does not meet the criteria for HLC)
To the best of their ability, the provider has clinically determined and documented the client appears able to receive HLC services (i.e. Client’s insurance covers HLC cost, client resources can afford HLC, physical limitations are not an issue, the client is able to travel to the HLC program, etc.)
The provider has clinically determined and documented the client would agree to, and comply with, receiving HLC services.
