How To: Use Sully.ai for Session Notes

Sully AI is a helpful tool that transcribes and summarizes your therapy sessions to support efficient and compliant documentation. While it can save time, it’s essential to use it responsibly and thoughtfully, your clinical judgment always takes precedence.

Step 1: Ensure Client Consent

Before recording any session with Sully:

  • Check the chart for a consent form:

    • New clients: This is included in their standard informed consent.

    • Existing clients: They may require a separate form titled Consent for AI-Assisted Note Taking.

  • No consent yet? Assign the "Consent for AI-Assisted Note Taking" form to the client.

  • Important: Do not record until consent is confirmed.



Step 2: Set Up and Record

Option 1: Use Sully via Web App

  1. Login at sully.ai.

  2. Pull up the client via scheduled appointment or select their name from your list.

  3. Test audio: Make sure your microphone and computer audio (client voice) are being captured.

If using Zoom with headphones or audio is not registering:

Headphones can prevent Sully from capturing the client’s voice. To work around this:

  • Open your Zoom session and disconnect your computer audio (click the arrow next to the microphone icon and select “Leave Computer Audio”).

  • Reconnect to the session by selecting “Join by Phone” in Zoom.

  • Use your Sully mobile app and choose the “Phone Call” feature to call into the Zoom session.

  • Confirm the correct call-in number and participant ID are entered to ensure you’re recognized in the session.

  • This method allows both your voice and the client’s voice to be captured clearly through your phone's microphone.

4. Once audio is confirmed, click Record in Sully and minimize the window to reduce distraction.

Option 2: Use the Sully Mobile App

  • Sully’s mobile app functions as an audio recorder and syncs with your Sully account.

  • Ideal for in-person sessions, phone sessions, or if you're having technical issues on your laptop.

  • Use the in-app call feature for phone sessions or as a workaround for Zoom (as described above).



Step 3: Review AI-Generated Note

After your session:

  • Click Finish Visit.

  • Sully will begin generating your note.

  • You can continue other tasks; the AI will notify you when the note is ready.

  • Click View Note to return to the client list and access the draft.



Step 4: Choose Note Format

Ruh has programmed Sully AI to support the following formats:

  • SOAP

  • DAP

  • Free Text (Org)

You can select your preferred default format in the Sully settings.



Step 5: Edit, Finalize & Transfer the Note

  • Always review the note for accuracy and completeness.

  • Sully may suggest a diagnosis or include interventions that don’t align with your actual care plan.

  • Key client details may be missing—e.g., “Client will be out of town for 3 months.”

  • Use the prompt feature to regenerate sections or add custom suggestions.

  • Sully notes may contain more detail than necessary—feel free to simplify. Sully also may make assumptions so please be sure to verify the details.

  • Reminder: You are the clinician of record. Finalize the note only when it accurately reflects your clinical assessment and tone.



Step 6: Send to the Client Chart & Lock

Once finalized:

  • Click Export to send the note to the client’s chart on the Ruh Care platform.

  • Manually open the chart to:

    • Ensure the note uploaded correctly.

    • Sign and lock the note as part of your documentation process.



Step 7: Delete Data from Sully

By default, Sully will auto-delete all session audio and notes after 7 days.

Best Practice: Delete immediately after locking your note in the platform to:

  • Avoid duplicate records

  • Reduce risk of exposure

  • Maintain the highest standard of confidentiality and client privacy

Sully is HIPAA-compliant, but because of the nature of recorded session data, extra caution is always encouraged.
